Toms River, NJ-8 week contract-Full-time flex day shifts (40 hours/week)-Start ASAP-Specific job functions may include, but are not limited to: -Assess and document pertinent, subjective, patient/client information:-Document on appropriate forms, as required, by utilizing the facility and/or institution. Follows the facility/institution policy of evaluation and documentation.-Utilizes the patient’s family, nurse and/or physician, when necessary, to obtain additional information.-Assess patient information provided by chart documentation for utilizing plan of care (i.e. vital signs, X-rays, EKGs).-Assess initial and on-going basic musculoskeletal and neurological testing (i.e. MMT, ROM, sensory, reflexes, etc.).-Develop a realistic objective and, measurable goals and utilize fellow therapists, physicians, nurses, and the patient with the development.-To develop, implement, and revise a plan of care consistent to the goals.-Assist the development, implementation, and instruction of a patient’s discharge plan.-To work with other therapists to ensure quality and the continuity of care.-To cooperate with the appropriate health care professionals while arranging the patient’s plan of care.-Inform the director when supplies or equipment are inoperable or deficient.-Assumes responsibility for personal growth and development by participation in continuing educations, workshops, seminars, in service programs, and orientation programs.-Adheres to the policies and procedures of the institution where assigned and of Favorite Healthcare Staffing.-To accept and fulfill obligations of scheduled assignments with Favorite Healthcare Staffing which have been requested and confirmed.-Seek assistance to regard to guidance or direction when necessary.
